Can lasers remove port wine stain birthmarks completely?

Although pulsed dye lasers are very effective in treating port wine stains (vascular birthmarks), it is difficult to determine how many treatments will be required for optimal results.  Much is contingent on the size of the birthmark, the depth in the skin and how much vascularity (blood vessels) there is.  Research shows that the location of the port wine stain also dictates how quickly the lesion will respond.

The goal with port wine stain birthmark removal is improvement rather than complete resolution.  It is not possible to say when we begin a laser birthmark removal program if we will be able to achieve 100% removal of the birthmak so starting with the expectation that significant improvement (rather than complete resolution) will be achieved is the smartest way to approach the treatments.  For port wine stains, fading of the birthmark with the newer generation of pulsed dye lasers offers patients a great way to minimize the appearance of port wine stains.

For this case above, the patient received 6 laser treatments.  8 - 12 treatments were recommended and the patient did not finish the program, although she did receive about 75% clearance of the birthmark.

How do I know if I am a good candidate for laser birthmark removal?

Birthmarks come in many shapes, sizes, colors, and may not appear until well after birth. They also come with a variety of names to distinguish which type of birthmark they are - cafe-au-lait, nevus of ota, etc.

For those birthmarks that are brown or darker in color (versus red/purple as found in vascular birthmarks), laser birthmark removal is an option.  Most practitioners agree that the gold standard medical laser for pigmented birthmark removal is a Q-Switched laser.  These lasers offer the best chance for improvement or resolution of the birthmark.

There are several types of birthmarks that may not be appropriate for laser treatments or require an evaluation by a dermatologist prior to seeking treatment. Apart from these situations, lasers offer a good option for the treatment of pigmented birthmarks.

Can a Port Wine Stain be treated with lasers?

Port Wine Stain is a term to describe a vascular birthmark.  These birthmarks are red/purple in color because of the concentration of blood vessels near the surface of the skin that make up the birthmark.  Because these blood vessels are superficial and are not needed, an effective treatment for the fading or resolution of port wine stain birthmarks is to close the superficial blood vessels using class IV medical lasers.  For example, pulsed dye lasers are very precise in targeting the hemoglobin found in the blood vessels of port wine stain birthmarks.  The thermal/heating effect causes the small capillaries to close altogether or diminish from view.  Port Wine Stain birthmarks are challenging to treat and may require up to 10 or 12 treatments; however, the use of Class IV medical lasers (pulsed dye lasers) is considered the "gold standard" for safe, effective treatment.

Does Laser Birthmark Removal work?

Laser birthmark removal can be effective if the correct technology (laser) is utilized.  Birthmarks come in various shapes, sizes, location, and pigmentation (color).  It is because of these factors that there is some unpredictability in the outcomes for laser birthmark treatment.  We believe in setting realistic expectations from the consultation and throughout the treatment plan.  Although most patients are satisifed with the expectation of moderate to significant fading, many of the birthmarks we treat resolve completely.  But, even with complete resolution, these lesions have the ability to return in the future, especially in the case of heavy and repeated sun exposure.  We typically set the expectation that pigmented birthmarks (cafe-au-lait etc.) can be resolved in 3-6 treatments and vascular birthmarks (port wine stains) will take more, usually 8-12 treatments.

The correct laser for treating birthmarks is crucial.  For brown pigmented birthmarks, the Q-Switched lasers are proven to be the most effective.  We currently have two different Q-Switched YAG lasers that have been provide excellent outcomes for cafe au lait birthmarks.  These lasers are safe for all skin types.  For red-based birthmarks that have a vascular component, we use the pulsed dye laser.  For more information on Celibre lasers and their uses, please go to http://www.celibre.com/equipment.aspx

There are a few categories of birthmarks that need to be addressed by a dermatologist or general/plastic surgeon for evaluation before lasers are considered.  However, the majority of benign cosmetic birthmarks are very treatable with class IV medical lasers.  When doing your research, find a practitioner that has experience in laser birthmark removal, has the appropriate laser for the type of birthmark you have, is knowledgeable about your skin type, and will provide realistic, honest expectations.
